Usually it's because the photo was too blurry, dark, or cut off, or the file was unclear to read. Try again with a sharper, well-lit photo taken straight-on, or upload the original PDF if you have it, and it should process correctly. Making sure all the text is in frame and in focus is the single best way to get a clean read.
